Abstract :
PURPOE: To determine whether intravitreal irrigation with Ca2+-free and Mg2+-free B Plu (Alcon Laboratory, Fort Worth, Texa) olution alter the adheivene between the retinal pigment epithelium and the retina of rabbit.
METHOD: Thirty-four eye of 34 Dutch pigmented rabbit underwent lenectomy and vitrectomy. ubequently, the vitreou cavity of 24 eye wa irrigated with Ca2+-free and Mg2+-free B Plu olution for 10 or 20 minute. The other 10 eye were irrigated with B Plu olution for 20 minute a control. To determine the adheivene between the retinal pigment epithelium and retina, a retinal detachment wa produced in 12 of the 34 eye. The apical urface of the retinal pigment epithelium and the photoreceptor outer egment were examined by canning electron microcopy. Retinal phyiology wa aeed by electroretinography and retinal morphology by light microcopy.
REULT: After retinal detachment wa produced, the number of cone heath on the apical urface of the retinal pigment epithelium after irrigation with Ca2+-free and Mg2+-free B Plu olution for 20 minute (33 ± 15, mean ± D) wa ignificantly le than the number of cone heath on the apical urface of the retinal pigment epithelium of eye after irrigation with B Plu olution for 20 minute (120 ± 50) or the number of cone heath on the apical urface of the retinal pigment epithelium of eye after 10 minute of irrigation with Ca2+-free and Mg2+-free B Plu olution (115 ± 49; P = .02). The b-wave amplitude in the eye irrigated with Ca2+-free and Mg2+-free B Plu olution for 20 minute were depreed compared with the b-wave in eye irrigated with B Plu olution for 20 minute on the firt potoperative day (P = .03). After the third potoperative day, there wa no ignificant difference in the b-wave (P > .06). Light microcopy demontrated no morphologic abnormalitie after the ue of both olution.
CONCLUION: Intravitreal irrigation with Ca2+-free and Mg2+-free B Plu olution for 20 minute altered the adheion between the retinal pigment epithelium microvilli and retinal outer egment and made the creation of retinal detachment le traumatic. Thee reult ugget that Ca2+-free and Mg2+-free B Plu olution may be of clinical value for the creation of an intentional retinal detachment for foveal tranlocation urgery
