Abstract :
PURPOE: A new urgical technique to tranlocate the macula wa ued to treat patient with ubfoveal choroidal neovacularization econdary to age-related macular degeneration (AMD).
DEIGN: Propective, interventional cae erie.
METHOD: Twenty-five eye of 25 patient underwent macular tranlocation with either circumferential or radial choriocleral outfolding uing three clip ize: 2-mm, 3-mm, and 4-mm. Potoperative photocoagulation wa performed on only thoe eye that had an extrafoveal choroidal neovacular membrane following urgery.
REULT: The urgery uccefully diplaced the fovea in 22 (88%) of the eye. The median potoperative foveal diplacement wa 1142 μm (range 0 to 3200 μm). Patient who had radial outfolding with 4-mm clip had the greatet diplacement of the fovea (range 1644 to 3200 μm median 1977 μm). The fovea wa uccefully diplaced to a location outide the choroidal neovacular membrane in 17 (68%) of the 25 eye. The bet-corrected viual acuity improved in 11 eye (median, 17 letter), remained unchanged in 4 eye, and decreaed in 10 eye (median, 12 letter). Viual acuity increaed by a median of 2 letter. The final bet-corrected viual acuity wa 20/64 in 3 eye; 20/80 in 3 eye; 20/100 in 4 eye; 20/126 in 4 eye; 20/200 in 4 eye; 20/250 in 4 eye; and 20/400 in 3 eye.
CONCLUION: Macular tranlocation with radial choriocleral outfolding uing 4-mm clip reulted in the bet foveal diplacement and improvement in viual function, and wa aociated with the leat amount of viion lo and complication. Further refinement are needed to make thi urgical procedure more predictable, and more reearch (randomized clinical trial) i needed to determine the role of macular tranlocation in the treatment of ubfoveal choroidal neovacularization in patient with AMD
