Abstract :
PURPOE: To report on the urgical outcome after at leat 12 month of follow-up of mini-trabeculectomy (without radial inciion) a an initial urgery.
METHOD: In a propective, intitutional tudy, mini-trabeculectomy wa performed a the initial urgical therapy on 41 eye with medically uncontrolled glaucoma of 41 conecutive patient aged 40 year or older. Mini-trabeculectomy conit of a 3-mm fornix-baed conjunctival flap, clerotomy at 1 mm from the limbu, and a clerocorneal tunnel without radial inciion. Of the 41 eye, four eye did not complete 12 month of follow-up. Of the remaining 37 eye, one eye underwent cataract extraction 5 month potoperatively and wa evaluated only for urgical complication. Thirty-ix patient with a mean age of 70.3 ± 7.4 tandard deviation year (range, 54 to 87) completed 12 month or more of follow-up.
REULT: In the 36 eye, mean preoperative intraocular preure wa 30.2 ± 9.3 mm Hg (range, 19 to 54) with 3.0 ± 1.2 hypotenive medication (range, 1 to 4). After potoperative mean follow-up of 25.0 ± 9.2 month (range, 12 to 43), intraocular preure wa at or below the planned target intraocular preure in 35 eye (97.2%; P < .0001) and mean intraocular preure wa 16.0 ± 2.8 mm Hg (range, 11 to 21) with 0.8 ± 1.0 medication (range, 0 to 3). Potoperative complication in the 37 eye included early potoperative aqueou leakage with moderately hallow or deep anterior chamber in two eye (5.4%) and rapid cataract progreion in one eye (2.7%). The four eye that were excluded from the tudy had intraocular preure of 10 to 16 mm Hg with 0 to 1 hypotenive medication, 3 to 9 month after urgery, repectively.
CONCLUION: Mini-trabeculectomy, which may offer clinical and technical advantage over the tandard trabeculectomy, wa generally efficaciou and relatively afe, baed on outcome oberved at a mean of 25 month follow-up. A controlled tudy i required to confirm thee obervation
