Abstract :
PURPOE: To decribe the characteritic of patient with AID and cytomegaloviru retiniti in the era of highly active antiretroviral therapy (HAART).
METHOD: Propective cohort tudy. Baeline (enrollment) data were compared between patient with newly diagnoed cytomegaloviru retiniti (incident cae) and thoe with previouly diagnoed cytomegaloviru retiniti (prevalent cae).
REULT: A of December 31, 2000, 45 incident and 200 prevalent cae had been enrolled. Among prevalent cae, the median time from cytomegaloviru retiniti diagnoi wa 2.9 year. Incident cae were more likely than prevalent cae to be women (35.4% v 15.3%, P = .001), African American (45.4% v 20.4%, P = .002), and uninured (29.6% v 7.6%, P < .001). Incident cae were le likely than prevalent cae to be on HAART (51.2% v 77.6%, P = .001) and to have had an immunologic repone to HAART (increae in CD4 + T-cell count to > 100 cell/μl) (12.2% v 57.5%, P < .001). The median CD4 + T-cell count at enrollment among incident cae wa 17 cell/μl and among prevalent cae wa 159 cell/μl (P < .001). Immune recovery uveiti had been diagnoed in 15.5% of the prevalent cae. ixty-three percent of prevalent cae were not on anticytomegaloviru therapy.
CONCLUION: There i a population of patient with previouly diagnoed and longtanding cytomegaloviru retiniti who have experienced immune recovery a a conequence of HAART and are no longer receiving anticytomegaloviru therapy. There are demographic difference between incident and prevalent cae that may reflect the evolution of the AID epidemic and difference in utilization of health care ervice.
