Abstract :
PURPOE : To report eight cae of pontaneou cloure of traumatic macular hole.
DEIGN : Conecutive obervational cae erie.
PATIENT AND METHOD : In a conecutive erie of 18 eye of 18 patient with traumatic macular hole, eight patient achieved pontaneou cloure of traumatic macular hole. Clinical record of the eight eye of eight patient were reviewed, together with the reult of optical coherence tomography performed in three eye.
REULT : All eight patient with pontaneou cloure of traumatic macular hole were male, with a mean age of 14.6 year (range, 11–21 year). The major caue of blunt trauma wa port-related accident. ix eye developed viual ymptom immediately after trauma and two eye 10 to 12 day later. In all eight eye, contact len biomicrocopy revealed a mall, full-thickne macular hole not complicated by epiretinal membrane, cuff of ubretinal fluid, or poterior vitreou detachment. The macular hole cloed pontaneouly 1 week to 4 month after trauma. All eight eye had viual acuity improvement with the final bet-corrected viual acuity of 0.5 or better in four (50%) eye. Optical coherence tomography in three eye revealed two ditinct abnormalitie. Two eye preented with acute foveal dehicence without involvement of the poterior vitreou cortex. The remaining eye howed at preentation perifoveal vitreou detachment with reidual vitreou adheion to the edge of updrawn fovea and developed releae of the vitreofoveal adheion at the time of hole cloure.
CONCLUION : pontaneou cloure of traumatic macular hole i not uncommon. An obervation for a period of up to four month may be a management of choice for traumatic macular hole. There may be clinically and pathogenetically two ditinct mechanim of traumatic macular formation; one type that caue immediate viual lo due to primary dehicence of the fovea, and the other type that lead to delayed viual lo due to dehicence of the fovea econdary to peritent vitreofoveal adheion.
