Abstract :
PURPOE: To analyze the feature of retinal detachment (RD) occurring after cataract urgery performed by Kelman phacoemulification (KPE) and to identify any potential correlation between KPE intraoperative complication and the feature of RD (incidence apart) a well a the final viual outcome.
DEIGN: Interventional conecutive cae erie.
METHOD: Retropective review of 114 eye of 114 conecutive patient with RD occurring after KPE and followed for 6 month or more after urgery.
REULT: Removal of poteriorly dilocated len fragment during KPE by par plana vitrectomy (PPV) wa aociated with a ignificantly horter time interval between KPE and RD (3.89 v. 15.7 month, P = .0044). Once RD occurred, no other tatitically ignificant correlation between it feature or the final viual outcome and KPE intraoperative complication (poterior capular rupture, vitreou lo, poteriorly dilocated len fragment) wa detected. The overall anatomic reattachment rate wa 94%. Only redetachment aociated with the development of proliferative vitreoretinopathy (PVR) wa ignificantly correlated with ultimate anatomic failure (P = .0036). A bet-corrected viual acuity of 20/60 or better wa achieved in 58 out of 114 eye (51%). Three variable were independently correlated with viual reult: more extenive RD (P = .0001), redetachment aociated with the development of PVR (P = .0029), and failure to identify retinal break (P = .0114).
CONCLUION: Poterior capular rupture and vitreou lo during KPE do not eem to affect the feature (incidence apart) or the final viual outcome of RD occurring afterward, except for a horter time interval between KPE and RD in eye that underwent PPV to remove poteriorly dilocated len fragment.
