Abstract :
PURPOE: To decribe an atypical cae of central nervou ytem and ocular paracoccidioidomycoe imulating ocular toxoplamoi in a pregnant woman with acquired immunodeficiency yndrome (AID).
DEIGN: Interventional cae report.
METHOD: Cae report.
REULT: A 25-year-old pregnant woman with AID, preented with a evere ocular inflammation in the right eye involving the choroid, retina, and the optic dik, which rapidly progreed to retinal detachment, iri neovacularization, and neovacular glaucoma. The left eye wa normal. Magnetic reonance imaging (MRI) howed a focal hypodene contrat-enhanced ring leion in the brain. erum antibody titer were negative for Toxoplama gondii, but the polymerae chain reaction wa poitive for the paraite in the vitreou ample. The patient reponded partially to pecific treatment for toxoplamoi, and there wa a mall reduction in ize of the brain leion. he progreed to a blind painful eye, which wa enucleated. Paracoccidioide brailieni wa found in the hitopathological tudie of the eye and oropharynx. With the diagnoi of dieminated ocular paracoccidioidomycoe, the patient wa treated with trimethoprim-ulfamethoxazole with a atifactory outcome and reduction in ize of the brain leion.
CONCLUION: Although ocular infection with ocular paracoccidioidomycoe i rare, thi diagnoi hould be conidered when invetigating ocular inflammation in a patient with AID.
