Abstract :
PURPOE: To develop an optical coherence tomography (OCT) claification ytem that monitor the repone of eye treated with photodynamic therapy (PDT) with verteporfin for ubfoveal choroidal neovacularization (CNV) from age-related macular degeneration (AMD). DEIGN: Retropective interventional cae erie. METHOD: Ninety eye (88 patient) with AMD and predominantly claic ubfoveal CNV treated with PDT uing verteporfin were identified by a laer log and retropectively reviewed. Optical coherence tomography and fluorecein angiography (FA) were performed before treatment and at ubequent follow-up examination in all eye. Optical coherence tomography finding were evaluated and compared with correponding FA. REULT: A five-tage OCT claification of eye treated with PDT wa created from the evaluation of 79 total eye (77 patient). tage I (two eye) i recognized within the firt week of treatment and demontrate an acute inflammatory repone with increaed ubretinal fluid. tage II (28 eye) repreent the retoration of a near-normal fovea contour with diminihed ubretinal fluid occurring 1 to 4 week after treatment. tage III (79 eye) occur between 4 to 12 week following treatment and i ubdivided into two categorie baed on the amount of ubretinal fibroi and fluid preent. tage IIIa (15 eye) contain a greater ubretinal fluid to fibroi ratio indicating an active CNV proce. Leion in tage IIIb (64 eye) le actively leak and have more prominent fibroi with minimal intraretinal fluid. Cytoid macular edema define a tage IV leion (11 eye). In tage V leion (19 eye) the ubretinal fluid reolve with thinning of the retina a well a fibroi merging with the retinal pigment epithelial layer (RPE). CONCLUION: Optical coherence tomography appear to be ueful in monitoring the retinal change that occur following PDT of CNV and may ait in undertanding the change oberved on angiography.
