Abstract :
Purpoe
To report the entity of partial detachment and folding of ubfoveal choroidal neovacularization (CNV) in age-related macular degeneration (ARMD).
Deign
Interventional cae report.
Method
Review of the feature of CNV detachment in two patient with CNV due to ARMD by contact len lit-lamp biomicrocopy, fluorecein angiography, optical coherence tomography, and three-dimenional confocal canning laer indocyanine green (ICG) angiography.
Reult
One patient out of approximately 300 (0.5%) ARMD patient treated by photodynamic therapy (PDT) developed partial CNV detachment and folding 6 week after the econd PDT treatment. Another patient out of approximately 100 (1.0%) ARMD patient treated by tranpupillary thermotherapy (TTT) developed partial CNV detachment and folding 6 week after the econd TTT treatment. The CNV were large (2,500 μm to 4,500 μm) and located between the retina and the retinal pigment epithelium. In each, thee finding were clearly viualized by lit-lamp biomicrocopy. Fluorecein angiography demontrated an aociated retinal pigment epithelium tear in one patient. Optical coherence tomography howed ditinctive feature and confocal canning laer ICG further delineated the detached folded CNV. The bet-corrected viual acuity improved in one patient from 20/80 to 20/40 and in the other from counting finger at 6 feet to 20/200 after the CNV detachment.
Concluion
Partial CNV detachment and folding repreent a unique, not previouly reported, and poibly favorable outcome of PDT and TTT. The low energy and electivity of thee treatment may explain thi phenomenon.
