Abstract :
Purpoe To evaluate the efficacy of photodynamic therapy with verteporfin in the management of choroidal neovacularization (CNV) aociated with angioid treak. Deign Retropective cae erie. Method Eleven eye of nine patient with ubfoveal or juxtafoveal CNV due to angioid treak underwent viual acuity teting, ophthalmic examination, color photography, and fluorecein angiography to evaluate the reult of photodynamic therapy with verteporfin. Retreatment of peritent CNV wa baed on criteria from the Treatment of Age-Related Macular Degeneration with Photodynamic Therapy Invetigation (TAP) except in one cae. Follow-up ranged from 5 to 28 month (mean, 17 month). Reult Nine of 11 eye had ubfoveal leion while two eye had juxtafoveal leion on initial examination. Converion from a choroidal neovacular membrane (CNVM) to a fibrou diciform leion following photodynamic therapy wa oberved in nine eye. Enlargement of the CNVM wa noted in even of thee eye by fluorecein angiography at final follow-up. Initial bet-corrected viual acuity (BCVA) ranged from 20/25 to counting finger (CF) (mean, 20/400; median, 20/200). Final BCVA ranged from 20/20 to CF (mean, 20/600; median, 20/400). even eye with ubfoveal CNVM had an initial BCVA of at leat 20/200 while only three eye maintained thi level or better at lat follow-up. In one patient with a juxtafoveal CNVM in one eye, viion decreaed from 20/25 to 20/400 with enlargement and fibroi of the CNVM and ubfoveal extenion. In the fellow eye a juxtafoveal CNVM wa initially treated and then retreated earlier than TAP criteria at 6 week. Viion improved to 20/20 and ha remained table 5 month after the initial treatment. Concluion Verteporfin for choroidal neovacularization–aociated with angioid treak doe not appear to ignificantly alter the coure of thi dieae with mot eye undergoing enlargement and diciform tranformation of the neovacular proce. However, aggreive management of thee patient with biomicrocopic and fluorecein angiographic examination and timely photodynamic therapy with early retreatment when indicated may be beneficial in certain cae
