Abstract :
Purpoe To evaluate the relationhip between glaucomatou tructural damage to the optic nerve and development of viual field lo with tandard automated perimetry (AP) and hort wavelength automated perimetry (WAP). Deign Cohort tudy. Method Patient with elevated intraocular preure and normal AP viual field were enrolled in thi propective tudy. tereo optic dik photograph, AP, and WAP viual field were obtained annually over a period of 4 or more year. Trained reader evaluated baeline and follow-up optic dik photograph for evidence of glaucomatou damage. tandard automated perimetry and WAP examination were evaluated according to previouly validated criteria for development of confirmed viual field change. Reult Two-hundred ninety-five ubject (479 eye) were enrolled. Following maked aement of tereo photograph by an optic dik reading center, 272 of the 479 eye were judged to have glaucomatou optic neuropathy at the time of tudy entry. Depending on the criteria employed, approximately 10% to 17.5% of all eye developed confirmed viual field lo for AP (converion). Of the converion, 75% to 80% had baeline glaucomatou optic dik damage, wherea normal and glaucomatou optic dik were equally divided (50%) among the nonconverion eye. Thi difference wa tatitically ignificant (P < .003). Depending on the criteria employed, 4% to 12% of the eye had confirmed WAP deficit at baeline, and 4% to 8% developed confirmed WAP defect at a follow-up examination. There wa a greater percentage of eye with a glaucomatou optic neuropathy in the group with WAP deficit (75%–100%) than for thoe eye in which WAP remained normal (45%–60%). ome of thee difference were tatitically ignificant (P < .05). Concluion A trong relationhip exit between glaucomatou optic dik damage at tudy entry and the ubequent development of a confirmed glaucomatou AP viual field defect. A higher percentage of glaucomatou optic dik were alo found in patient with WAP deficit at baeline and in thoe who later developed WAP deficit. Thee finding upport the premie that a glaucomatou optic dik i predictive of the ubequent development of glaucomatou viual field lo.
