Optical coherence tomography for evaluating diabetic macular edema before and after vitrectomy

Purpoe To report the ue of optical coherence tomography (OCT) for evaluation of diffue diabetic macular edema (DME) before and after vitrectomy. Deign Interventional cae erie. Method A retropective tudy wa made of 15 conecutive eye of 13 patient that had vitrectomy for diffue DME and OCT preoperatively and potoperatively. In even eye of ix patient (group 1), vitrectomy wa performed becaue of vitreomacular traction oberved on biomicrocopy or OCT. In the other eight eye of even patient (group 2), vitrectomy wa performed for DME not reponive to laer photocoagulation, with no vitreomacular traction on biomicrocopy or OCT. Reult Mean ± tandard deviation (D) follow-up after vitrectomy wa 18 ± 10 month (range, 6 to 33 month). In group 1, mean ± D retinal thickne decreaed ignificantly from 661 ± 181 μm preoperatively to 210 ± 32 μm at the end of follow-up (P = .018). Median bet-corrected viual acuity (BCVA) improved from 20/100 before urgery (range, 20/250 to 20/50) to 20/80 at the end of follow-up (range, 20/250 to 20/25; P = .046). In one eye in group 1, vitreomacular traction wa only oberved on OCT and not on biomicrocopy. In group 2, mean ± D retinal thickne decreaed from 522 ± 103 μm preoperatively to 428 ± 121 μm at the end of follow-up (P = .2). Median BCVA wa 20/100 before vitrectomy (range, 20/320 to 20/63) and 20/200 at the end of follow-up (range, 20/250 to 20/63; P = .78). Concluion Vitrectomy wa beneficial in eye with diffue DME combined with vitreomacular traction but not in eye without traction. Optical coherence tomography allowed diagnoi of ubtle vitreomacular traction and provided precie preoperative and potoperative aement of macular thickne.
