Abstract :
Purpoe To report the rate of retinal detachment after vitrectomy for retained intravitreal len material after phacoemulification uing pecific vitrectomy technique deigned to minimize retinal detachment. Deign Conecutive, interventional cae erie. Method Retropective chart review of 100 conecutive eye (one urgeon) of 100 patient undergoing vitrectomy for retained len material after phacoemulification and followed up for 3 month or longer unle an outcome event had occurred. Vitrectomy technique employed to minimize the frequency of retinal detachment included inducing poterior vitreou detachment with maximal vitreou removal before phacofragmentation to avoid vitreou trauma, len fragment debulking before fragmentation, ue of low energy with high apiration during removal of retained len material, and intraoperative indirect ophthalmocopic evaluation of the retinal periphery with cleral indentation to diagnoe and treat intraoperative retinal break. The main outcome meaure included prevalence of coexiting retinal detachment in eye with retained len material, incidence of retinal detachment or retinal break after vitrectomy for removal of retained len material, and final viual acuity. Reult The prevalence of previtrectomy retinal detachment wa 4%; the incidence of potvitrectomy retinal detachment wa 4%; the final viual acuity wa 20/40 or better in 53%. One patient had a retinal break recognized during vitrectomy and wa treated with retinocryopexy, but potoperative retinal detachment developed from a eparate break. Three other were treated during vitrectomy for retinal break (including two with known preexiting break) and did not have any retinal detachment. Poor previtrectomy viual acuity (hand motion) wa a rik factor for potvitrectomy retinal detachment. Concluion The rate of retinal detachment reported after vitrectomy for retained len material after phacoemulification can be minimized to approximately the rate expected with cataract extraction complicated by vitreou lo by employing tandard urgical technique. Higher rik eye may benefit from more frequent potvitrectomy examination.
