Abstract :
Purpoe To report two cae of progreive outer retinal necroi occurring in immunocompetent individual after treatment with corticoteroid for preumed optic neuropathy. Deign Obervational cae report. etting Univerity–baed tertiary eye hopital. Method Retropective review of exiting clinical record. Reult Two patient were treated empirically with ytemic corticoteroid for upected inflammatory papillopathy. ubequently, both were diagnoed with necrotizing herpetic retiniti with feature of progreive outer retinal necroi. Anterior chamber paracentei confirmed varicella-zoter infection. Both patient were human immunodeficiency viru negative; one patient with rheumatoid arthriti wa taking etanercept. Both became completely blind in one eye depite intenive treatment with antiviral medication intravenouly and intravitreally. Concluion Progreive outer retinal necroi i not confined to patient with underlying evere immunodeficiency, uch a acquired immune deficiency yndrome. Initial treatment of acute, unexplained viion lo with ytemic corticoteroid may lead to catatrophic viual lo in patient with evolving necrotizing herpetic retinopathy.
