Abstract :
Purpoe
To invetigate the anatomic and viual outcome in patient with initial anatomic ucce after macular hole urgery and with at leat 5 year of follow-up.
Deign
Retropective, noncomparative, conecutive cae erie.
Method
Medical record of all patient who underwent urgery for idiopathic full-thickne macular hole by two urgeon (W.E.., H.W.F.) at the Bacom Palmer Eye Intitute between January 1, 1991, and December 31, 1996, were reviewed. All patient who had initial anatomic ucce with macular hole urgery and who had 5 year or more of follow-up potoperatively were included in the tudy. Main outcome meaure included the rate of macular hole reopening and viual acuity outcome.
Reult
eventy-four eye of 66 patient with a median age of 68.0 year (range, 45.0–86.8 year) were identified. The median duration of macular hole wa 6.0 month (range, 1.1–93.8 month), and the median duration of follow-up after macular hole urgery wa 91.0 month (range, 60.0 to 114.8 month). The hole reopened in 9 eye (12%) during the follow-up interval; 6 of thee eye underwent reoperation, and the hole cloed in 4 of 6 (67%). Preoperative viual acuity ranged from 20/50 to 20/400 (mean, 20/129; median, 20/100). In the 62 eye that underwent cataract extraction (CE) after macular hole urgery, CE wa performed at a median of 13.9 month after macular hole urgery. Patient achieved their bet potoperative viual acuity at a median of 28.5 month after macular hole urgery. Bet potoperative viual acuity ranged from 20/20 to 20/400 (mean, 20/36; median, 20/30). Viual acuity at lat follow-up ranged from 20/25 to counting finger (mean, 20/56; median, 20/40). At lat follow-up, 43 eye (58%) had a viual acuity of 20/40 or better, and 57 (77%) had an improvement in viual acuity of 3 or more nellen line compared with their preoperative acuity.
Concluion
Macular hole cloure and viual acuity improvement after initially ucceful macular hole urgery perit at follow-up of 5 year and longer in the majority of patient; delayed viual acuity improvement i not attributable to cataract urgery alone.
