Abstract :
Purpoe To determine the probability of converting from pigment diperion yndrome to pigmentary glaucoma. Deign Retropective community-baed tudy of all newly diagnoed cae of pigment diperion yndrome or pigmentary glaucoma. Method ubject were patient newly diagnoed with pigment diperion yndrome or pigmentary glaucoma from 1976 to 1999 in Olmted County, Minneota. Criteria for pigment diperion yndrome were two of three ign: midperipheral, radial iri tranillumination defect; Krukenberg pindle; heavy trabecular mehwork pigmentation. Criteria for pigmentary glaucoma were pigment diperion yndrome and two of three finding: intraocular preure (IOP) greater than 21 mm, optic nerve damage, or viual field lo. Kaplan-Meier urvival curve were ued to determine the probability of converion to pigmentary glaucoma. Reult A total of 113 patient were newly diagnoed with pigment diperion yndrome over the 24-year period. Of thee, 9 peron developed pigmentary glaucoma or elevated IOP requiring therapy. The probability of converting to pigmentary glaucoma wa 10% at 5 year and 15% at 15 year. An additional 23 patient were found to have pigmentary glaucoma at their initial examination. The mean age at diagnoi of pigmentary glaucoma wa 42 ± 12 year; 78% of patient were male, wherea 58% of patient with pigmentary diperion yndrome glaucoma were male. The mot ignificant rik factor for converion to pigmentary glaucoma wa an IOP greater than 21 mm Hg at initial examination, wherea age, refractive error, and family hitory of glaucoma were not correlated with converion. Concluion The rik of developing pigmentary glaucoma from pigment diperion yndrome wa 10% at 5 year and 15% at 15 year. Young, myopic men were mot likely to have pigmentary glaucoma. An IOP greater than 21 mm Hg at initial examination wa aociated with an increaed rik of converion.
