Abstract :
Purpoe
To evaluate the enitivity and pecificity of optic dik examination performed uing the Moorfield regreion analyi (MRA) of the Heidelberg Retina Tomograph (HRT) in differentiating normal from glaucomatou eye.
Deign
Retropective cro-ectional tudy.
Method
Five hundred and nineteen patient were included in the tudy for a total of 193 normal eye, 213 with upected glaucoma (primary open-angle glaucoma [POAG]), and 113 with POAG. The intervention conited of optic dik imaging by mean of HRT I. A mean of three repeated image wa analyzed uing verion 2.01 oftware. The optic dik wa claified a “normal/outide normal limit” on the bai of the MRA. The viual field wa examined uing the D 24 II program (Humphrey perimeter), with a glaucomatou viual field being defined on the bai of an abnormal Glaucoma Hemifield Tet and a tatitically ignificant corrected pattern tandard deviation greater than 2 dB. The reult obtained with MRA were compared with thoe obtained uing the multivariate dicriminant analyi (MDA) provided in the HRT I. The main outcome meaure were enitivity, pecificity, and poitive and negative predictive value of HRT examination.
Reult
The enitivity and pecificity of the HRT-MRA examination were, repectively, 74% and 94% (83% and 75% with MDA) when the patient with upected POAG were excluded from the analyi; the figure were 74% and 85%, and 41% and 94% (83% and 64%, and 60% and 75% with MDA) when the ame patient were included a being normal or having POAG.
Concluion
In a broad clinical etting including normal ubject, patient with upected POAG, and POAG patient, the HRT-MRA howed a high degree of diagnotic accuracy. The MRA wa le enitive but more pecific than MDA. Greater diagnotic ability may be added by HRT-MRA examination than by HRT-MDA to tandard POAG diagnotic tudie.
