Abstract :
Purpoe To compare plama homocyteine concentration among patient with exfoliation yndrome, exfoliative glaucoma, normal-tenion glaucoma, and normal control ubject without vacular or inflammatory ocular dieae or glaucoma. Deign Cro-ectional tudy. Method We teted 25 patient with exfoliation yndrome, 50 with exfoliative glaucoma, 25 with normal-tenion glaucoma, and 24 control ubject. Fating plama homocyteine concentration were meaured by fluorecence polarization immunoaay. Patient uing vitamin upplement or medication known to alter erum homocyteine were excluded. Reult Homocyteine level were higher in both exfoliation group compared with control (exfoliation yndrome: P = .003; exfoliative glaucoma: P = .009); level in normal-tenion glaucoma were higher than but not ignificantly different from thoe in control (P = .2). Hyperhomocyteinemia wa preent in 16 of 25 (64%) exfoliation yndrome patient, 28 of 50 (56%) exfoliative glaucoma patient, 13 of 25 (52%) normal-tenion glaucoma patient, and 7 of 24 (29.2%) control (P = .005). Multiple logitic regreion analye comparing exfoliation yndrome and exfoliative glaucoma patient with control indicated that elevated plama homocyteine concentration wa a ignificant rik factor for exfoliation yndrome, in both thoe patient (odd ratio per 1.0 μmol/l increae in plama homocyteine concentration = 1.47; 95% confidence interval [CI] = 1.08–2.0) and in exfoliative glaucoma patient (odd ratio = 1.3; 95% CI = 1.07–1.6). Although exfoliative glaucoma and normal-tenion glaucoma patient were not ignficantly different with repect to hyperhomocyteinemia, logitic regreion modeling of exfoliative glaucoma v normal-tenion glaucoma patient howed that an increaed homocyteine concentration wa a ignificant rik factor for exfoliation yndrome in the preence of glaucoma (odd ratio per 1.0 μmol/l increae in homocyteine = 1.2, 95% CI = 1.0–1.4). Thee relationhip were not affected by adjutment for potential confounding due to ex, hitory of hypertenion, or other factor. Concluion Elevated plama homocyteine, a rik factor for cardiovacular dieae, i more common in exfoliation yndrome and exfoliative glaucoma patient than healthy control. Patient with exfoliation yndrome may benefit from meaurement of homocyteine level
