Abstract :
Purpoe
To analyze the indication for and outcome of penetrating keratoplaty (PKP) performed by reident urgeon at both county hopital and Veteran Affair facilitie.

Obervational cae erie
Method
Retropective review of 79 eye from 61 patient undergoing PKP from January 1, 1997, through December 31, 2001, to determine indication for urgery and outcome, including graft clarity and final viual acuity.
Reult
There were 52 (65.8%) primary and 27 (34.2%) repeat tranplant performed. Follow-up after urgery averaged 21.9 month. Indication for PKP included failed graft, 23 (29.1%), bullou keratopathy, 17 (21.5%), keratoconu, 16 (20.3%), corneal car, 15 (19.0%), corneal perforation from infection, 5 (6.3%), and Fuch endothelial dytrophy, 3 (3.8%). Twenty-one eye had pre-exiting ocular dieae limiting final acuity. Overall, 51 graft (64.6%) remained clear. More primary than repeat graft remained clear (75% v 44.4%, P = .012) and achieved 20/40 or better final acuity (30.8% v 11.8%, P = .05). Excluding failed graft or eye with limited viual potential, final acuitie achieved were: 19 (47.5%) 20/40 or better, 18 (45%) 20/50 to 20/150, 3 (7.5%) 20/200 or wore. Graft remaining clear by indication were: failed graft, 9 (39.1%); keratoconu, 14 (87.5%); bullou keratopathy, 13 (76.5%); corneal car, 10 (66.7%); corneal perforation, 2 (40%); Fuch, 3 (100%). Eye achieving final acuity of 20/40 or better by indication were: keratoconu, 9 (56.2%); failed graft, 3 (13%); bullou keratopathy, 3 (17.7%); corneal car, 2 (13.3%); corneal perforation, 0 (0%); Fuch, 2 (66.7%).
Concluion
County hopital and Veteran Affair facilitie provide a challenging ubet of patient for penetrating keratoplaty. Failed graft wa the leading indication for tranplantation for our population. Graft clarity and final viual acuity varied by indication for tranplantation. Reident urgeon can achieve favorable reult for penetrating keratoplaty performed at thee venue, epecially for primary tranplant.
