Abstract :
Purpoe
To tudy the poible aociation between clear corneal inciion with or without placement of a uture during cataract extraction and potoperative endophthalmiti.
Deign
Retropective, comparative, cae-controlled tudy.
Method
Thirty-eight patient treated for culture-poitive, acute potcataract urgery endophthalmiti, and 371 randomly elected control patient who underwent uncomplicated cataract urgery in the referring community were tudied. Inciion type and ue of uture during cataract urgery of endophthalmiti patient were compared with the control. The data were analyzed uing logitic regreion method.
Reult
Of the 38 patient with endophthalmiti, 17 patient (45%) had clear corneal inciion and 21 patient (55%) had a cleral tunnel inciion. In 371 control, 76 patient (20%) had clear corneal inciion and 295 patient (80%) had cleral tunnel inciion. Clear corneal inciion wa aociated with a threefold greater rik of endophthalmiti than wa cleral tunnel inciion (Odd Ratio, 3.36, 95% Confidence Interval = 1.67 to 6.78). The type of inciion wa ignificant (χ2 = 11.53, P = .0007); a clear corneal inciion wa more frequently aociated with endophthalmiti. A ubgroup analyi revealed that the preence or abence of a uture wa not ignificant (χ2 = 1.31, P = .2524).
Concluion
In thi retropective, cae-controlled tudy, clear corneal inciion were found to be a tatitically ignificant rik factor for acute potcataract urgery endophthalmiti when compared with cleral tunnel inciion
