Abstract :
Purpoe To report on the ucceful cloure of a peritent chronic macular hole uing indocyanine green (ICG) during the econd urgical approach. Deign Interventional cae report Method A patient with a long tanding macular hole (18 year) underwent par plana vitrectomy with peeling of the internal limiting membrane (ILM). While no dye wa ued during the firt operation, ICG (0.05%, Pulion) wa applied during the econd procedure. The light ource wa turned off immediately after the ICG application until the dye wa wahed out after a period of 15 econd. Tiue harveted during the econd urgical approach wa prepared for electron microcopy. Reult After the failure of the firt operation, area of incomplete ILM peeling could be tained uing ICG and a thorough ILM removal wa achieved. Potoperatively, the macular hole wa cloed, a confirmed by optical coherence tomography. Viual acuity increaed from 20/200 to 20/50. No viual field defect were een potoperatively. There were only few cellular element adherent to the retinal urface of the ILM a een during electron microcopy. Concluion In cae where tractional force due to incomplete ILM removal are conidered reponible for the peritence of a macular hole, ICG i ueful to identify area of unpeeled ILM. Under certain afety precaution no advere effect on functional outcome wa een.
