Abstract :
Purpoe To determine whether difference in baeline leion ize and viual acuity might explain differing reult found in three different leion compoition (predominantly claic, minimally claic, and occult with no claic) among three placebo-controlled, randomized clinical trial evaluating photodynamic therapy with verteporfin (Viudyne, Novarti AG), alo termed verteporfin therapy, in patient with ubfoveal choroidal neovacularization (CNV) due to age-related macular degeneration (AMD). Method Exploratory analye were conducted in patient with predominantly claic or minimally claic leion at enrollment in the Treatment of AMD with Photodynamic Therapy (TAP) Invetigation and in AMD patient with occult with no claic CNV in the Verteporfin In Photodynamic Therapy (VIP) Trial. Baeline characteritic of patient among thee three leion compoition were compared. In addition, multiple linear regreion modeling wa ued to explore the effect of baeline leion ize, viual acuity, and leion compoition on mean change in viual acuity from baeline to 24 month. Reult At baeline, the mean ize of predominantly claic leion (3.4 dik area) wa maller than that of minimally claic (4.7 dik area) and occult with no claic leion (4.3 dik area). In the multiple linear regreion model of individual leion compoition, there wa a ignificant treatment-by-leion-ize interaction for minimally claic and occult with no claic leion, but not for predominantly claic leion. Interaction between treatment and baeline viual acuity wa not ignificant for any leion compoition. mall verteporfin-treated leion lot le viion than large verteporfin-treated leion in each leion compoition. In the multiple linear regreion model that included all leion compoition, leion ize wa a more ignificant predictive factor for the magnitude of treatment benefit than either leion compoition or viual acuity. maller (4.0 dik area or le) minimally claic and occult with no claic leion had imilar viual acuity outcome to thoe oberved in predominantly claic leion. Concluion Baed on exploratory analye, leion ize in the TAP Invetigation and VIP Trial wa an important predictor of the magnitude of treatment benefit with verteporfin therapy in occult with no claic and minimally claic leion compoition. In patient with AMD, treating maller rather than larger neovacular leion, regardle of leion compoition, likely will reult in a better level of viual acuity.
