Abstract :
Purpoe
To determine the afety of par plana vitrectomy in eye containing a treated poterior uveal melanoma.
Deign
Interventional cae erie.
Method
Retropective cae erie of patient with poterior uveal melanoma who underwent par plana vitrectomy. Complication, vitreou cytology, local tumor control, and metatai were aeed.
Reult
Nine patient met tudy criteria. Tumor were treated with 125I plaque radiotherapy (even patient) or tranpupillary thermotherapy (two patient). Vitrectomy wa performed for vitreou hemorrhage (five patient), macular pucker (two patient), macular hole (one patient), and rhegmatogenou retinal detachment (one patient). Vitrectomy wa performed at a mean of 24.7 month (range, 7–47 month) after melanoma treatment. Diperion of tumor cell at vitrectomy wa not oberved in any patient. Melanoma cell were detected in the vitreou apirate in one of even cae examined cytologically. Thi patient had intratumoral and vitreou hemorrhage before plaque radiotherapy, underwent combined vitrectomy/cataract extraction, and developed intraocular tumor diemination 56 month after vitrectomy. No other patient developed intraocular tumor diemination. At mean follow-up of 24 month (range, 3–63 month) after vitrectomy, none of the nine patient developed ytemic metatai.
Concluion
Par plana vitrectomy rarely may lead to intraocular tumor diemination, although the rik of thi complication i probably low if the tumor ha been treated and ha reponded to therapy before vitrectomy. Vitrectomy hould be approached with caution if a vitreou hemorrhage i preent, epecially if the hemorrhage occurred before tumor treatment, a thi may eed tumor cell into the vitreou cavity.
