Abstract :
Purpoe To ae the coure of the repone to patching treatment of moderate amblyopia and to ae factor predictive of the repone in children 3 year old to younger than 7 year old. Deign Multicenter, randomized clinical trial comparing patching and atropine (one of the amblyopia treatment tudie). Method A total of 209 children 3 year old to younger than 7 year of age with amblyopia in the range of 20/40 to 20/100 from the patching treatment arm of thi trial were treated with patching of the ound eye from 6 hour per day up to all waking hour. Follow-up examination were performed at 5 week, 16 week, and 6 month. The primary outcome meaure wa viual acuity in the amblyopic eye at 6 month. Reult After 5 week of treatment, mean amblyopic eye acuity improved from baeline by 2.2 line. For patient with baeline acuity of 20/80 or 20/100, a greater number of hour of precribed patching wa aociated with greater improvement in the firt 5 week (P = .05). However, thi relationhip wa not preent when baeline acuity wa 20/40 to 20/60 (P = .57). At 6 month, viual acuity wa improved from baeline by a mean of 3.1 line, with the amount of improvement no longer related to the number of hour patching precribed at baeline (P = .93). Among the 157 patient improving at leat 3 line from baeline, 15% achieved their maximum improvement by 5 week and 52% by 16 week. None of the demographic or clinical factor aeed wa predictive of the repone to treatment. Concluion In the treatment of moderate amblyopia, a beneficial effect of patching i preent throughout the age range of 3 year old to younger than 7 year old and the acuity range of 20/40 to 20/100. At 6 month, the amount of improvement appear to be imilar when 6 hour of daily patching are initially precribed v a greater number of hour.
