Abstract :
Purpoe To review our initial experience with radial optic neurotomy a treatment for retinal vein occluion. Deign Interventional cae erie. Method Patient population: five patient (four with central retinal vein occluion and one with hemiretinal vein occluion). Intervention Par plana vitrectomy with radial optic neurotomy wa performed in each cae. Main outcome meaure Bet-corrected viual acuity, preence of macular edema, perfuion tatu, and time to venou phae of the angiogram were reviewed retropectively. Reult Mean preoperative viual acuity wa 4/200. Preoperatively, the vein occluion wa perfued in one (20%), nonperfued in one (20%), and indeterminate in three (60%). Mean follow-up time wa 4.5 month. Mean potoperative viual acuity wa 20/400 at lat follow-up. Four patient (80%) had improvement in viual acuity and one (20%) worened. Two patient (40%) improved to 20/80 potoperatively. In four cae (80%), dik congetion improved and intraretinal hemorrhage reaborbed more quickly than would be expected without treatment. Time to the venou phae of fluorecein angiography improved lightly in three cae (60%) potoperatively. Perfuion tatu a determined by fluorecein angiography wa not ignificantly altered potoperatively. One patient (20%) had reolution of macular edema potoperatively a hown by volumetric optical coherence tomography. One patient developed choroidovitreal neovacularization and one developed iri neovacularization potoperatively, both of which reponded to panretinal photocoagulation. Concluion Radial optic neurotomy may improve viual acuity in eye with central retinal vein occluion, although choroidovitreal neovacularization from the neurotomy ite can occur. Further tudy i needed to determine it role in the management of central vein occluion.
