Meta-analyi of plama homocyteine, erum folate, erum vitamin B12, and thermolabile MTHFR genotype a rik factor for retinal vacular occluive dieae

Purpoe To ae the role of plama total homocyteine (tHcy) level, erum folate and vitamin B12level, and homozygoity for the thermolabile methylenetetrahydrofolate reductae genotype (TT) a rik factor for retinal vacular occluive dieae. Deign Meta-analyi of literature. Method A MEDLINE earch wa performed to identify all publihed cae–control tudie of plama tHcy level, erum folate and vitamin B12 level, and TT genotype in peron with retinal vacular occluive dieae. Main outcome meaure included calculation of plama tHcy, erum folate, and erum vitamin B12 tandard difference and odd ratio (OR) of TT genotype between cae and control. Reult In total, 614 patient with all type of retinal vein occluion had higher plama tHcy level than 762 control ubject (tandard difference, 0.867; 95% confidence interval [CI] = 0.735, 0.999; P < .001). Plama tHcy level were alo higher in 154 patient with retinal artery occluion compared with 358 control ubject (tandard difference 1.174; 95% CI = 0.947, 1.402; P < .001). erum folate, but not vitamin B12 level, were lower in 287 patient with retinal vacular occluion than in the ame number of control ubject (tandard difference, 0.508; 95% CI = 0.340, 0.675; P < .001; and −0.060; 95% CI = −0.024, 0.104; P = .474, repectively). imilar proportion of 690 patient with retinal vein occluion and 2754 control ubject demontrated the TT genotype (OR = 1.332; 95% CI = 0.995, 1.783; P = .054) a did 152 patient with retinal artery occluion and 435 control ubject (OR = 1.716; 95% CI = 0.977, 3.014; P = .060). Concluion Retinal vacular occluion i aociated with elevated plama tHcy level and low erum folate level, but not erum vitamin B12 level and TT genotype. Until a propective multicenter trial i undertaken, plama tHcy level and erum folate level hould be determined in patient with retinal vacular occluion, and dietary upplementation with low doe of folate and vitamin B12 hould be conidered for affected peron.
