Abstract :
Purpoe To evaluate peritency of pharmacotherapy in primary open-angle glaucoma upect (glaucoma upect) treated with latanoprot and timolol. Deign Retropective, cohort tudy uing the Protocare cience managed care databae; approximately 3 million member in commercial health maintenance organization and preferred provider organization and in Medicare rik plan. Method Patient 20 year of age or older beginning therapy between January 1, 1997, and June 30, 2002, with latanoprot or timolol monotherapy were included. Patient mut have been continuouly enrolled and not undergone glaucoma urgery in the year preceding the index precription fill and had glaucoma upect diagnoe before and after the index date. Precription refill record for all ocular hypotenive were extracted through June 30, 2002. The two outcome meaure were (1) dicontinuation of index drug, and (2) either dicontinuation or change in index drug. Changing therapy wa defined a witching to or adding another ocular hypotenive. Rate of dicontinuation and dicontinuation/change were compared uing Cox regreion model. Reult In all, 1,474 patient met the incluion criteria. Latanoprot wa precribed for 583 patient (40%) and timolol for 891 (60%). Compared with latanoprot, thoe treated with timolol were 39% more likely to dicontinue and 27% more likely to dicontinue/change therapy (P < .001 for both comparion). At 12 month, 39% of patient receiving latanoprot and 25% of thoe treated with timolol had not dicontinued therapy; no dicontinuation or change in therapy wa een in 30% and 18%, repectively. Concluion Latanoprot-treated glaucoma upect demontrated ignificantly greater peritency than did patient treated with timolol. The reaon for thi difference and it impact on intraocular preure control and dieae progreion require further reearch.
