Abstract :
Purpoe To examine the impact of previou phototherapeutic keratectomy (PTK) on the outcome of ubequent penetrating keratoplaty (PK) in patient with tromal corneal dytrophie. Deign Retropective, cro-ectional, clinical ingle-center tudy. Method Patient Population: Fifteen patient (21 eye) age 39.9 ± 11.4 year. Incluion Criteria: Primary homologou PK performed in phakic patient with granular or macular dytrophy; no ue of combined urgical procedure; defined graft ize and technique. The tudy group compried eight eye of five patient, PK performed 3.7 ± 2.3 year after PTK. The control group (no previou PTK) compried 13 eye of 10 patient. In both group, 38% had granular and 62% had macular dytrophy. Intervention Procedure: Phototherapeutic keratectomy wa performed uing a 193 nm excimer laer. All PK were alo performed uing thi laer, with trephination uing a metal mak. ubjective refractometry (trial lene), tandard keratometry (Zei ophthalmometer), and corneal topography (Tomey TM-1) were performed preoperatively, 6 month after PK, and after firt and econd uture removal (1.1 ± 0.2 year; 1.6 ± 0.2 year). Main Outcome Meaure: Keratometric, topographic net atigmatim, and refractive cylinder; keratometric and topographic central power; bet-corrected viual acuity (BCVA); urface regularity index (RI), urface aymmetry index (AI), potential viual acuity (PVA). Reult Refractive power and atigmatim, BCVA, and PVA value did not differ ignificantly between the two group at any time-point; RI tended to be better in the tudy group after firt uture removal (P = .05). Concluion Preceding PTK doe not appear to impair the outcome of ubequent penetrating keratoplaty in tromal corneal dytrophy patient.
