Culture-proven endogenou endophthalmiti: clinical feature and viual acuity outcome

Purpoe To invetigate clinical feature and viual acuity outcome aociated with endogenou endophthalmiti. Deign Retropective, obervational cae erie. Method Twenty-one eye of 21 patient treated at Bacom Palmer Eye Intitute for culture-proven endogenou endophthalmiti between 1996 and 2002 were reviewed. Reult Patient were followed a mean of 3 month (range, 1 to 12 month). Fungal iolate occurred in 13 eye (62%), gram-poitive iolate in 7 (33%), and gram-negative iolate in 1 (5%). Twelve patient (57%) were hopitalized at the time of diagnoi and 6 patient (29%) died within 2 month of diagnoi. Initial treatment included tap and injection of intravitreal medication in 10 eye (48%) and par plana vitrectomy with injection of intravitreal medication in 11 eye (52%). Final viual outcome were obtainable for 18 eye (two patient died within 10 day of diagnoi, and one patient wa lot to follow-up). Eight (44%) of thee 18 eye achieved a viual acuity of 20/400 or better and 10 (56%) of 18 eye achieved a viual acuity wore than 20/400, including 3 that were either enucleated or evicerated. Three eye with Apergillu endophthalmiti had wore viual outcome than eye with either Candida (P = .036) or bacterial endophthalmiti (P = .024). Concluion Compared with publihed erie of potoperative or pot-traumatic endophthalmiti, patient with endogenou endophthalmiti are more likely to have fungal iolate with a predominance of Candida albican. Endogenou endophthalmiti i generally aociated with high mortality and poor viual acuity outcome, particularly when caued by more virulent pecie uch a Apergillu.
