Abstract :
Purpoe To compare the ubjective viual experience of cataract patient during phacoemulification and intraocular len implantation under topical anetheia (TA) v retrobulbar anetheia (RA). Deign Propective, randomized, controlled trial. Method Three hundred ix cataract patient eligible for phacoemulification and intraocular len implantation were randomized to receive either TA or RA during urgery by one of three urgeon. The urgeon were familiar with both anethetic technique and operated on the patient uing the technique to which the patient were randomized. A maked interviewer conducted in-peron interview with the patient uing a tandardized quetionnaire about their intraoperative viual experience and their reaction to their viual experience between 30 minute and 4 hour after the urgery. Reult Two patient (one in each group) had intraoperative poterior capule rupture and were excluded from analyi. There wa no tatitically ignificant difference between TA (n = 154) and RA (n = 150) group, except that more male compared with female had TA (P = .03). More patient who had TA reported perception of light (P < .001) and color (P < .001) and a change (either increae or decreae) in light brightne during the coure of the urgery (P < .001). There wa no tatitically ignificant difference in the proportion of patient who aw movement, flahe, intrument, or the urgeon or medical taff during the operation and who found their viual enation frightening in the two group. However, 10.4% in the TA group and 9.3% in the RA group found their viual experience frightening. Concluion More patient undergoing cataract urgery under TA compared with RA reported perception of light, color, and a change in light brightne. A ignificant proportion of patient in both group found the viual experience frightening.
