Abstract :
Purpoe
To introduce a cae of POEM (polyneuropathy, organomegaly, endocrinopathy, monoclonal gammopathy, and kin change) yndrome with bilateral cytoid macular edema (CME).
Deign
Interventional cae report.
Method
Temporal change of erum vacular endothelial growth factor (VEGF) level and macular thickne were evaluated.
Reult
A 53-year-old woman with POEM yndrome wa referred complaining of long-tanding impaired viion. Typical CME demontrating petal-like dye pooling wa oberved in both eye. After ubtenon adminitration of triamcinolone acetonide (TA), par plana vitrectomy, and intraocular TA injection in the right eye, macular thickne decreaed. Intraocular VEGF level were <31 pg/ml at the vitrectomy and intraocular TA injection, wherea erum the VEGF level wa >2000 pg/ml throughout the follow-up period. In the untreated left eye, macular thickne altered related to erum VEGF level.
Concluion
Baed on the low intraocular VEGF level and erum VEGF-related change of macular thickne, elevated erum VEGF might be a caue of CME in POEM yndrome.
