Abstract :
Purpoe
To track the relative frequency and explore poible rik factor of fungal compared with bacterial endophthalmiti after corneal tranplantation.
Deign
Cae-comparion tudy neted in a urveillance regitry.
Method
We examined, among recipient who underwent urgery from January 1991 through December 2003, demographic and eye-banking characteritic of potkeratoplaty endophthalmiti cae that were reported to the Eye Bank Aociation of America by U eye bank. Potential predictor of fungal compared with bacterial endophthalmiti were examined uing multiple logitic regreion.
Reult
Of 234 reported cae of potkeratoplaty endophthalmiti reported by U eye bank, 46 cae (19.7%) were fungal, and 130 cae (55.6%) were bacterial. Concordant culture of the reidual donor corneocleral rim or preervation medium occurred ignificantly more often with fungal than bacterial endophthalmiti (P = .009). After the introduction of Optiol-G, the odd of bacterial relative to fungal endophthalmiti decreaed by 77% (95% confidence interval, 44% - 91%). After adjutment for the preervation method and other eye-banking variable, the odd of fungal endophthalmiti wa 3.4 (95% confidence interval, 1.6 - 7.4) time that of bacterial endophthalmiti, when donor corneal preervation wa 4 day or longer.
Concluion
The frequency of potkeratoplaty bacterial endophthalmiti compared with fungal endophthalmiti fell after widepread ue of a corneal preervation medium upplemented with gentamicin and treptomycin. Further improvement in corneal preervation procedure are needed.
