Abstract :
Purpoe
To compare the effectivene of par plana vitrectomy (PPV) and removal of the internal limiting membrane (ILM) with modified grid laer photocoagulation in diabetic macular edema.
Deign
Randomized, comparative, interventional tudy.
Method
In thi propective tudy, 24 eye of 12 patient with bilateral diabetic macular edema were evaluated. PPV with removal of the ILM wa performed at random in one eye of 12 patient (ILM group), and a ingle eion of modified grid laer photocoagulation wa performed in the fellow eye (grid group). Main outcome meaure were the foveal thickne meaured with optical coherence tomography, preoperative, and potoperative viual acuitie. Mann-Whitney U and Wilcoxon tet were ued in tatitical analyi.
Reult
All patient were followed up for 6 month. In the ILM group, mean foveal thickne wa 439.2 ± 106.5 μm preoperatively and 219.8 ± 63.2 μm potoperatively (P = .002). In the grid group, mean foveal thickne wa 407 ± 100.2 μm preoperatively and 378.5 ± 141.6 μm potoperatively (P = .433). A mean decreae in foveal thickne wa found to be 219.4 ± 127.6 μm in the ILM group and 28.5 ± 90.5 μm in the grid group (P = .001). In the ILM group, bet-corrected logMAR viual acuity wa 0.75 ± 0.41 preoperatively and 0.53 ± 0.41 potoperatively (P = .006). In the grid group, bet-corrected logMAR viual acuity wa 0.59 ± 0.27 preoperatively and 0.49 ± 0.27 potoperatively (P = .058). Viual acuity improved by 2 or more line in ix eye (50%) in the ILM group and in three eye (25%) in the grid group. Viual acuity remained table in ix eye (50%) in the ILM group and in nine eye (75%) in the grid group.
Concluion
PPV with ILM removal appear to be more effective than a ingle eion of modified grid laer photocoagulation in the treatment of diabetic macular edema. Further tudie with a large number of patient are required for a more reliable concluion.
