Abstract :
Purpoe To analyze the clinical preentation, microbiological profile, and treatment outcome in cae of nocardial endophthalmiti after cataract urgery. Deign Retropective, obervational cae erie. Method etting: Tertiary referral hopital. patient: Retropective analyi of 24 cae of culture-proven exogenou nocardial endophthalmiti following cataract urgery between January 2000 and December 2003. main outcome meaure: Viual acuity and anatomic outcome. Reult Viual outcome wa poor in mot patient, with 79% (19/24) of patient obtaining a final viual acuity of hand motion or wore and 46% (11/24) of patient with no light perception. Initial preenting viual acuity wa trongly aociated with final viual outcome (P = .0026). There wa a tatitically ignificant trend favoring better viual outcome in patient who preented early after the onet of ymptom (P = .01). The majority of the eye (23/24) had an early onet with predominately anterior egment involvement. Wound infection wa noted in 46% (11/24) of the patient at preentation. Clinical feature included cleral abce, cotton ball exudate in the anterior chamber, fluffy exudate on the corneal endothelium, nodular exudate on the iri, and hypopyon. Among the pecimen cultured, the highet poitivity wa een with the anterior chamber apirate (15/16 = 93.75%), which wa ignificantly higher than the vitreou apirate cultured (1/22 = 4.54%) (P = .001). Concluion Nocardia endophthalmiti after cataract urgery i an aggreive dieae with poor viual prognoi. Early diagnoi and treatment with good viual acuity at preentation may be aociated with better viual outcome
