Accommodative Intraocular Len Tilting

Purpoe To report an unuual complication of accommodative intraocular len (IOL) implantation after clear len extraction for hyperopia correction. Deign Obervational cae report. Method A 48-year-old woman underwent clear len exchange for the correction of moderate hyperopia. A Crytalen Model AT-45 Accommodating Poterior Chamber IOL (AT-45 IOL) wa implanted to allow optimal ditance and near viion. At the 3-week follow-up appointment, he complained of monocular diplopia. Ocular examination howed an increaed atigmatim, cauing decreaed viual acuity. cheimpflug Pentacam Image and Wave Front Analyi upported the diagnoi of IOL tilting. Reult IOL repoitioning wa unucceful becaue of fibroi of the haptic. It wa neceary to replace AT 45 IOL with a monofocal acrylic ulcu-fixated IOL. Concluion Control of capular fibroi hould be a major concern, epecially in thi type of IOL. Accommodating IOL exchange appear to be a afe alternative to manage thi complication.
