Effect of Change in elf-Reported Viion on Cognitive, Affective, and Functional tatu and Living Arrangement Among the Elderly

Purpoe To tudy effect of change in elf-reported viion on functional tatu, cognition, depreive ymptom, and living arrangement. Deign Longitudinal analyi of houehold urvey data. Method A total of 6234 ample peron oberved in the tudy of Aet and Health Dynamic Among the Oldet Old (AHEAD) 1995 were followed in 1998, 2000, and 2002 or until death or ample attrition. Effect of change in elf-reported viion and other factor were aeed by mean of ordinary leat-quare and logitic regreion with panel data method. Main outcome meaure were limitation of intrumental activitie of daily living (IADL), activitie of daily living (ADL), and other, cognition, depreive ymptom, and living arrangement. Reult A decline from excellent/good viion to fair/poor near and ditance viion had tatitically ignificant effect on everal IADL limitation, and ome ADL and other limitation. Larget effect were for driving (OR for no limitation: 0.55, P = .003), managing money (OR: 0.61, P < .001), and preparing hot meal (OR: 0.61, P < .001). Onet of fair-poor near viion increaed the likelihood of onet of at leat one IADL (OR for no limitation: 0.71, P < .01) and ADL (OR: 0.74, P = .003) limitation. Onet of legal blindne reulted in a 78% increae in the likelihood of an IADL limitation (OR for no limitation: 0.22, P < .001). Effect of viion decline on cognition and depreive ymptom were tatitically ignificant but mall. Decline in viion increaed the probability of nuring home reidence. Concluion Viual impairment ha major impact on functional tatu. Preventing viion lo i likely to appreciably improve the functioning of elderly peron.
