Abstract :
Purpoe Thi tudy ought to determine the influence of relaxing retinotomy (RR) inciion upon urgical outcome in the repair of recurrent retinal detachment (RD) attributable to proliferative vitreoretinopathy (PVR). Deign Retropective, conecutive, nonrandomized, ingle-center erie. Method Eighty-one eye with recurrent RD attributable to PVR were retropectively reviewed. Excluion criteria were giant retinal tear, uveiti, trauma, proliferative diabetic retinopathy, and age under 18 year. A total of 52 eye underwent RR at the time of urgery (64.2%); 29 eye were repaired without thi technique. Perfluorocarbon ga (n = 34) or ilicone oil (n = 47) wa ued a potoperative tamponade. tatitical analye were performed uing the Fiher exact tet. Reult Eye that received RR had ignificantly higher rate of anterior PVR (P = .009). Eye receiving ilicone oil for potoperative tamponade had wore baeline characteritic compared with thoe receiving ga. The ue of RR in eye receiving ga tamponade had no marked influence on the initial anatomic outcome, with recurrent retinal detachment occurring in five of 14 eye that received an RR and even of 20 eye that did not receive an RR (P = .62). Eye in which ilicone oil wa ued a a potoperative tamponade had a ignificantly lower rate of recurrent RD requiring additional urgery when RR wa employed (one of 38 eye) when compared with eye that did not receive an RR (three of nine eye, P = .02). Ultimately, urgical reattachment wa attained in all eye except one. Eye that received ga tamponade without RR had ignificantly better median viion (P = .008). Concluion urgical management of PVR often reult in ultimate retinal reattachment. An RR inciion doe not appear to influence initial anatomic repair when ga tamponade i ued after vitrectomy urgery for PVR. However, RR may increae the initial urgical ucce rate in eye receiving ilicone oil tamponade for PVR. In eye undergoing RR for the treatment of evere PVR, the ue of ilicone oil may increae the initial rate of reattachment compared with the ue of ga tamponade.
