Cytomegaloviru Retiniti After Intravitreou Triamcinolone in an Immunocompetent Patient

Purpoe To report the cae of a 75-year-old man with diabete who developed cytomegaloviru (CMV) retiniti after intravitreou injection of triamcinolone acetonide (IVTA). Deign Obervational cae report. Method Review of medical record. Reult A 75-year-old man with diabetic macular edema developed arcuate retinal whitening after IVTA. A preumptive diagnoi of viral retiniti wa made, and a vitrectomy wa performed. Polymerae chain reaction of the vitreou wa poitive for CMV DNA. An infectiou dieae conultant found no ign of ytemic CMV infection, and laboratory examination revealed that the patient wa HIV negative. The patient reponded well to intravitreal ganciclovir and oral valganciclovir, but when therapy wa dicontinued, the retiniti recurred and CMV DNA wa again detected in the vitreou. The retiniti once again reponded to antiviral therapy. Concluion CMV retiniti can occur after local immunouppreion with IVTA. Clinician hould be aware of thi rare complication of IVTA.
