Abstract :
Purpoe
The tudy’ purpoe wa to determine the effect that lack of capular upport for a nonutured poterior chamber (PC) intraocular len IOL ha on 1-year potoperative viion in patient uffering vitreou preentation (VP) during extracapular cataract urgery.
Deign
The tudy wa a propective, long-term clinical trial.
Method
The tudy wa conducted at 19 Department of Veteran Affair medical center. Patient having VP during cataract urgery with ufficient capular upport for a nonutured PC IOL were randomized to receive either a PC IOL (230 patient) or an anterior chamber (AC) IOL (208 patient). Patient having inufficient capular upport (IC) for a nonutured PC IOL (143 patient) were propectively followed in the ame fahion. Bet-corrected viual acuity (BCVA) at 1 year wa obtained by a maked, certified examiner.
Reult
The IC group had coniderably fewer patient with BCVA of 20/40 or better at 1 year (P < .0001) than the randomized PC IOL group (66.7% v 91.1%) with a trend (P = .04) for having fewer patient than the randomized AC IOL group (66.7% v 79.0%). The IC group had ignificantly poorer rating of viion at 1 year by tudy rater, and the patient themelve than either of the randomized VP group.
Concluion
The lack of capular upport for the placement of a nonutured PC IOL after VP during extracapular cataract urgery or the everity of the complication of VP aociated with lack of capular upport would appear to caue viual problem beyond that related to the type of intraocular len implanted.
