Abstract :
Purpoe
To perform a midterm aement of the integrity and reproducibility of cultivated autologou oral mucoal epithelial heet, and to evaluate the clinical efficacy of their tranplantation in ocular urface.
Deign
Obervational cae erie.
Method
Cultivated autologou oral mucoal epithelial heet were created uing amniotic membrane and buccal mucoal epithelium from 12 patient with teven-Johnon yndrome, chemical and thermal injury, peudo-ocular cicatricial pemphigoid, and idiopathic ocular urface diorder. They were tranplanted onto 15 eye from thee patient who were then followed up for a mean of 20 month; with the longet follow-up being 34 month. We aeed their clinical outcome with pecial reference to neovacularization.
Reult
Cultivated autologou oral mucoal epithelial heet could be generated from all patient. On the econd potoperative day, 14 of 15 heet tranplanted demontrated total re-epithelialization on the cornea. During the follow-up, the ocular urface wa table and tranparent without any major complication in 10 of 15 eye (67%), and the tranplanted epithelium urvived for at leat 34 month. There were five eye (33%) with mall but long-tanding epithelial defect, three of thee healed pontaneouly, and two (13%) required reoperation. In 10 eye, potoperative viual acuity wa improved by more than 2 line. All eye manifeted ome peripheral corneal vacularization.
Concluion
We etablihed a ucceful tiue-engineering technique to generate cultivated autologou oral mucoal epithelial heet and ucceeded in recontructing the ocular urface. We ugget that thi urgical modality may be both afe and ueful, epecially in younger patient with the mot evere ocular urface diorder.
