Abstract :
Purpoe
To compare the urgical outcome of bilateral lateral rectu receion and unilateral rece-reect (RR) procedure on the nondominant eye for the patient of exotropia with a dominant fixating eye.
Deign
Propective randomized comparative clinical trial.
Method
One hundred twenty-four patient of exotropia with an invariably fixating eye were enrolled to thi tudy. Patient were aigned randomly to two group, thoe who underwent bilateral lateral rectu (BLR) receion (BLR group) or unilateral RR procedure on the nondominant eye (RR group); urgical outcome were compared. An outcome wa conidered atifactory if there wa between 10 prim diopter of exophoria/tropia and 10 prim diopter of eophoria/tropia at 6 month after urgery.
Reult
In the BLR group, 28 of the 58 patient (48.3%) had a atifactory outcome, and 30 patient (51.7%) had recurrence. There wa no cae of overcorrection in the BLR group. In the RR group, 55 of the 66 patient (83.3%) had a atifactory outcome; 6 patient (9.1%) had recurrence, and 5 patient (7.6%) were overcorrected (P < .001, Fiher’ exact tet). All overcorrected patient in the RR group had poor tereoacuity and contant exotropia before the operation. The cumulative probability of urgical ucce wa ignificantly higher in the RR group than in the BLR group (P = .012, log rank tet).
Concluion
In the patient with exotropia with a dominant eye, the unilateral RR procedure reulted in a better outcome than BLR receion urgery. But, the overcorrection rate wa ignificantly higher in the unilateral RR procedure group, epecially in thoe patient with a poor preoperative tereopi tatu and contant exotropia.
