Abstract :
Purpoe
To determine the prevalence of a family hitory (FMH) of inflammatory bowel dieae (IBD) in patient with idiopathic ocular inflammation (OI).
Deign
Retropective obervational tudy.
Method
Medical record of idiopathic OI patient een between January 1995 and June 2005 in the Univerity of Illinoi Uveiti Clinic were creened for a FMH of IBD. FMH included firt, econd, and third degree relative.
Reult
727 (32.8%) patient had idiopathic OI. 26 of 727 (3.6%) and 10 of 727 (1.4%) reported a FMH of ulcerative coliti (UC) and Crohn’ dieae, repectively. 16 (2.2%) patient with a FMH of indeterminate coliti were excluded from ubequent comparion. The prevalence of a FMH of IBD among patient with idiopathic OI wa 3- to 15-fold higher than the prevalence of UC and Crohn’ in the general population (0.055% to 0.27%). Keratoiriti wa more common and granulomatou dieae le frequent in patient with a FMH of IBD (P = .014 and .019). Patient teted 26.3% with a FMH of IBD, and 66.7% of patient teted with IBD themelve, were HLA-B27 poitive (odd ratio [OR] 5.6, 95% confidence interval [CI] 0.77 to 45.8), compared with 40.1% of patient with neither (OR 1.87, 95% CI 0.61 to 6.83).
Concluion
A FMH of IBD may be an independent rik factor for the development of OI. In contrat to the ubgroup of patient with a peronal hitory of IBD, HLA-B27 may not be an appropriate marker in the ubgroup of patient with merely a FMH of IBD, emphaizing the importance of invetigating other immunologic factor that may predipoe patient with IBD and their relative to OI.
