Abstract
Purpoe
To determine whether higher-order aberration can explain the monocular diplopia reported by a patient.
Deign
Obervational cae report.
Method
A patient complaining of monocular diplopia wa examined with the Hartmann-hack aberrometer to determine if the higher-order wavefront aberration could account for the diplopia. The patient had a mild cortical cataract, and meaurement were made before and after lenectomy. In addition, the retinal image wa imulated uing Zernike polynomial.
Reult
pherical aberration (0.20 μm for 4-mm pupil) and econdary atigmatim (−0.12 μm) were increaed in the eye. The imulated retinal image had a double configuration that wa approximately the ame a the ubjective image reported by the patient. After cataract urgery, the diplopia diappeared, and the pherical aberration and econdary atigmatim were coniderably decreaed.
Concluion
The monocular diplopia probably temmed from the combined effect of pherical aberration and econdary atigmatim caued by the cortical cataract.
