Abstract :
Purpoe To evaluate the autologou tranlocation of peripheral choroid and retinal pigment epithelium (RPE) in 45 eye of 43 patient with age-related macular degeneration (AMD). Deign Propective nonrandomized tudy. Method All patient had viual lo due to AMD (n = 5 claic membrane, n = 14 occult, n = 2 mixed, n = 16 pigment epithelial detachment (PED), n = 5 ubretinal hemorrhage, n = 3 geographic atrophy). After extraction of the neovacular complex, an autologou peripheral full-thickne explant of RPE, Bruch membrane, and choroid wa tranlocated from the midperiphery to the macula. Reult Preoperative ditant viual acuity ranged from 20/800 to 20/40. Reading viion ranged from 1.4 logarithm of reading acuity determination (logRAD) to 0.5 logRAD (0.04 to 0.32 nellen equivalent). Reviion urgery wa required in 22 eye a a reult of proliferative vitreoretinopathy (PVR), retinal detachment, macular pucker, or vitreou hemorrhage. In eight patient, the patch wa renewed. At ix month, ditant viual acuity ranged from light perception to 20/50 (increae of 15 letter in four eye). Reading viion ranged from 1.4 to 0.4 logRAD. Viual outcome wa unrelated to the type of AMD. Vacularization of the tranplant wa viible on indocyanine green (ICG) angiography in 40 of 42 eye. In mot patient, autofluorecence of the pigment epithelium wa coincident with revacularization of the graft. Fixation on the patch wa poitively related to viual acuity. Concluion Autologou tranlocation of a full-thickne tranplant of choroid and RPE uually reult in a vacularized and functioning graft. Vacularization wa even achieved in patient with geographic atrophy. Fixation tability and microperimetry before the patch tranlocation may be helpful in electing patient who will profit from urgery.
