Abstract :
Purpoe To tudy the ue of prophylactic fourth-generation fluoroquinolone antibiotic, gatifloxacin and moxifloxacin, and bacterial enitivity in cae of acute potoperative endophthalmiti following cataract urgery. Deign Retropective, conecutive, obervational cae erie. Method Forty-two eye of 42 patient with acute endophthalmiti occurring within ix week after cataract urgery were identified. All patient were een in a referral vitreoretinal practice over a two-year time interval. The number of patient uing prophylactic gatifloxacin or moxifloxacin and reult of bacterial culture and enitivity to all fluoroquinolone antibiotic were recorded. Reult Thirty-one of 42 eye (74%) were treated with perioperative gatifloxacin or moxifloxacin and 24 eye (57%) were continuouly taking one of thee antibiotic at the time of diagnoi. Nineteen eye (45%) had a poitive bacterial culture. The mot frequent organim iolated wa coagulae-negative taphylococcu. enitivitie were performed for 14 gram-poitive organim, and enitivitie to ciprofloxacin (50%), ofloxacin (44%), levofloxacin (46%), gatifloxacin (38%), and moxifloxacin (38%) were noted. Five organim were reitant to gatifloxacin and moxifloxacin with a minimum inhibitory concentration of 8 μg/ml. All gram-poitive organim were enitive to vancomycin. Median viual acuity improved from hand motion to 20/40 at lat follow-up. Concluion Acute endophthalmiti can develop after cataract urgery depite the prophylactic ue of fourth-generation fluoroquinolone antibiotic. Gram-poitive organim cauing acute endophthalmiti are frequently reitant to all fluoroquinolone, including a ignificant number of cae reitant to gatifloxacin and moxifloxacin.
