Abstract :
Purpoe
To identify baeline demographic and clinical factor aociated with undergoing penetrating keratoplaty (PK) in a propective cohort of 1,065 keratoconu patient followed for eight year in the Collaborative Longitudinal Evaluation of Keratoconu (CLEK) tudy.
Deign
Multicenter, propective, obervational cohort tudy.
Method
We report the rate of PK over eight year and baeline factor predictive of PK in 1,065 patient who, at the time of tudy enrollment, had not undergone PK in either eye.
Reult
Eighty-two percent of patient completed the eight-year cloe-out viit. Twelve percent (126 of 1,065) had PK in one (9.3%) or both eye (2.5%). Baeline factor aociated with increaed likelihood of PK included younger age, teeper keratometric value, wore viual acuity, corneal carring, poorer contact len comfort, and poorer viion-related quality of life. The percent of eye undergoing PK wa 15% for patient 40 year old and younger, 28% for eye with a teep keratometric value greater than 52 diopter, 33% for viual acuity le than 20/40, and 24% for eye with corneal carring.
Concluion
The CLEK tudy confirmed previou report of the increaed likelihood of PK aociated with corneal carring, teeper keratometry value, poorer viual acuity, and poorer contact len comfort. The CLEK tudy i among the firt to report an increaed rik of PK aociated with younger age, wore viion-related quality of life, and flatter contact len fit. Knowledge of thee factor i beneficial to clinician in patient education and may be ueful in dieae management.
