Abstract
Purpoe
To evaluate the afety and efficacy of combined phacoemulification, intraocular len implantation, and trabeculectomy with mitomycin C for the management of uveitic complication.
Deign
Retropective cae-control tudy.
Method
We conducted a retropective review of the record of 23 conecutive eye with chronic noninfectiou uveiti (uveitic group) and 43 nonuveitic eye (control group) that had received primary phacotrabeculectomy. Mitomycin C wa ued in all the uveitic eye. Conidering the high preoperative intraocular preure (IOP) of the uveitic group, nonuveitic eye that had a preoperative IOP of ≥20 mm Hg or that had been given two or more medication were included in the control group. All patient were followed for at leat one year. The main outcome meaure were potoperative viion, IOP control, complication, and acute uveiti relape rate.
Reult
Viual outcome of the uveitic group wa imilar to the control group. In the uveitic group, the ucce rate of IOP control (91.3% at one year, 84.8% at two year) wa favorable but wa ignificantly lower than in the control group (P = .0423). Complication were comparable between the group. Primary urgical failure in the uveitic group wa aociated with the potoperative acute uveiti attack. In the uveitic group, the acute uveiti attack rate howed no change after urgery (P = .283).
Concluion
With adequate inflammation uppreion, phacotrabeculectomy with mitomycin C i an effective and afe therapeutic option for the management of econdary cataract and glaucoma in uveitic eye. A lower urgical ucce rate of the uveitic group might be attributable to the potoperative inflammation recurrence.
