Abstract :
Purpoe
To compare the efficacy and viual reult of phacoemulification v manual uturele mall-inciion extracapular cataract urgery (IC) for the treatment of cataract in Nepal.
Deign
Propective, randomized comparion of 108 conecutive patient with viually ignificant cataract.
Method
etting: Outreach microurgical eye clinic. patient: One hundred eight conecutive patient with cataract were aigned randomly to receive either phacoemulification or IC. intervention Cataract urgery with implantation of intraocular len. main outcome meaure: Operative time, urgical complication, uncorrected and bet-corrected viual acuity (BCVA), atigmatim, and central corneal thickne (CCT).
Reult
Both urgical technique achieved excellent urgical outcome with low complication rate. On potoperative day 1, the group had comparable uncorrected viual acuity (UCVA) (P = 0.185) and the IC group had le corneal edema (P = 0.0039). At ix month, 89% of the IC patient had UCVA of 20/60 or better and 98% had a bet-corrected viual acuity (BCVA) of 20/60 or better v 85% of patient with UCVA of 20/60 or better and 98% of patient with BCVA of 20/60 or better at ix month in the phaco group (P = 0.30). urgical time for IC wa much horter than that for phacoemulification (P < .0001).
Concluion
Both phacoemulification and IC achieved excellent viual outcome with low complication rate. IC i ignificantly fater, le expenive, and le technology dependent than phacoemulification. IC may be the more appropriate urgical procedure for the treatment of advanced cataract in the developing world.
