Abstract :
Purpoe To evaluate the effectivene of radial optic neurotomy (RON) for central retinal vein occluion (CRVO) in patient ≤50 year of age (group 1) v thoe >50 (group 2). Deign Propective, interventional, comparative cae erie. Method The tudy included 43 conecutive patient with CRVO and preoperative viual acuity (VA) ≤0.70 logarithm of minimal angle of reolution (logMAR). All patient underwent par plana vitrectomy and RON at the naal border of the optic dik. VA and optical coherence tomography (OCT) finding were recorded preoperatively and at one, ix, and 12 month potoperatively. Reult ytemic hypertenion, diabete, and open-angle glaucoma were tatitically ignificantly more prevalent in group 2 (P < .05). One patient in group 1 had hyperhomocyteinemia, and had another antiphopholipid yndrome. Fifty percent of patient in group 1 gained ≥3 line of Early Treatment Diabetic Retinopathy tudy (ETDR) viion, v 33% in group 2. Mean final VA wa 0.5 logMAR VA in group 1 v 0.8 in group 2 (P = .04). Foveal thickne decreaed ignificantly in both group (P < .001). Ten patient (55.6%) in group 1 and 13 (54.2%) in group 2 developed retinochoroidal collateral. Concluion Underlying ytemic dieae doe not eem to be an important factor in the pathogenei of CRVO in younger patient, and thrombophilia wa preent in only 11% of patient in thi age group. RON yielded better functional reult in younger patient, although functional improvement remained limited in thoe with low baeline VA.
