Purpoe To decribe the ucceful treatment with ytemic poaconazole of a erie of Fuarium ocular infection refractory to other antifungal agent. Deign Retropective, interventional cae erie. Method We identified three patient from three different center who received diagnoe of Fuarium keratiti and who received ytemic poaconazole after their infection failed to repond to maximal tolerated medical and urgical therapy. All patient received multiple ytemic, intracameral, and topical antifungal agent, which were ineffective in controlling their infection. Reult Two patient were contact len wearer. Two patient developed endophthalmiti. The infection of all three patient failed to repond to ytemic and/or topical voriconazole treatment. One patient could not tolerate ytemic voriconazole, and another experienced no marked improvement. The third dicontinued treatment for both reaon. All patient were treated with oral poaconazole and experienced rapid reduction of intraocular inflammation and pain, a well a reolution of the infection without additional intervention. Concluion Deep Fuarium keratiti i difficult to treat and carrie a high rik of progreion to endophthalmiti. Poaconazole, which exhibit excellent tiue penetration and demontrate efficacy in the treatment of ytemic Fuarium infection, wa ucceful in treating three cae of pan-reitant keratiti and/or endophthalmiti.
